Consent for participation in Decisioning Research Project

Decisioning Research Project

We've noticed a gap in the leadership development space, and with your help, we'd like to fill it with stories, resources and insights. We will be conducting one-hour interviews with leaders from across industries, collecting qualitative data through recording and transcribing your interviews. Our findings will be analyzed to create new knowledge that we will share in a variety of formats for interested audiences. Since your stories and insights, including direct quotes, may be shared, your participation in this research project requires your consent. Access to your interview recordings and transcript will be limited to two primary research investigators, Cristine Saxon and Brenda Frank, plus two others to assist in interviewing and data analysis, Doug Knight and Spencer Medford.

Any intellectual property created using the interview content belongs solely to the Decisioning Research Project. If you decide at any time in the future that you do not want your content used by the Decisioning Research Project, you can submit your decision in writing to Cristine Saxon, Brenda Frank or through this website and we will delete all identifiable content.

By providing your consent you agree to hold the project and researchers harmless from all losses, damages, liabilities, deficiencies, actions, judgments, interest, awards, penalties, fines, cost or expenses of whatever kind arising from the use of your interview content.

How to Give Your Consent

If you have any questions, contact the project member who connected with you to arrange an interview. To give your consent, fill in the Consent form below which provides us with your consent to use the transcript/recording of your interview as part of the Decisioning Research Project to inform the overall findings, including your agreement to the direct use of your content, including quotes or stories. We will contact you individually if we plan to use a direct quote or story. Once we have received your consent via the form below, we will contact you to set up your one-hour interview.
